Instructions for effecting a wire transfer of stock to United Hospital Fund

1. Draft a letter/memo of authorization to your broker, with the following language:

This letter represents authorization for transfer of [# of shares] shares of [name of stock] from [name of donor] to the United Hospital Fund of New York.  This gift is given as an unrestricted gift to United Hospital Fund's annual campaign.
 
Please forward an acknowledgment of this gift to:

1. Stock certificates should be sent directly to the United Hospital Fund.  Stock powers, if required, should be sent under separate cover to the United Hospital Fund (contact information below).
