Performing Provider System Projects: Tackling the Health Needs of Communities

Since early 2014, health care providers across the state have collaborated in the development of Performing Provider Systems (PPSs) in order to participate in New York Medicaid’s Delivery System Reform Incentive Payment (DSRIP) program. A key goal of the program is to reduce avoidable hospital use by 25 percent over five years by transforming the delivery system to better coordinate care and meet the needs of beneficiaries in the community.
On December 22, 2014, a total of 25 emerging Performing Provider Systems applied to participate in the DSRIP program. These Performing Provider Systems are collectively responsible for improving the health outcomes of approximately six million Medicaid beneficiaries. This data brief provides a snapshot of the projects selected by the Performing Provider Systems as part of their application.
