Improving Transitional Care by Involving Family Caregivers
The National Health Policy Forum, an independent Washington-based group that provides information and analysis to congressional and federal agency staff, held a session on October 28, 2011, titled “Preventing Hospital Readmissions: How Can Care Transitions for Medicare Beneficiaries Be Improved?” Among the panelists was Carol Levine, director of the Fund’s Families and Health Care Project, whose presentation (available below) was on the Fund’s Transitions in Care Quality Improvement Collaborative (TC-QuIC).
