6,333 seniors were surveyed in New York State, including:
     5,764 from New York City
     282 from Long Island
     287 from Upstate

59% are white, non-Hispanic
57% live alone
99% have primary health insurance
25% have diabetes
     36% of Hispanics have diabetes
32% have heart disease
     37% of white, non-Hispanics have heart disease
27% have fallen
46% rate their health as "fair/poor"
     58% of those with diabetes rate their health as "fair/poor"
     55% of those who have fallen in the past 12 months rate their health as "fair/poor"
      60% of those with heart disease rate their health as "fair/poor"
15% are taking 10+ medications
     26% of people with heart disease are taking 10+ medications
8% are living with 4 or more chronic conditions
     27% of people with diabetes are living with 4 or more chronic conditions
     21% of people with heart disease are living with 4 or more chronic conditions
30% get the recommended level of physical activity
23% have seen their primary medical provider 7 or more times in the past 12 months
     32% of people with heart disease have seen their primary medical provider 7 or more times in the past 12 months
