To increase access to primary medical care services for psychiatry patients through the use of volunteer patient navigators.

Many patients diagnosed with mental illnesses do not receive regular primary care.  This project will increase patients’ access to primary care services through the use of volunteer Patient Navigators, who will be an integral part of the care team at a new primary care clinic located at the psychiatric clinic.  Navigators will assist patients in scheduling appointments, make reminders and follow-up calls, and provide escort where needed.  Navigators will also educate patients on what to expect from a medical visit, provide information cards that teach patients how to formulate questions for the doctor, and conduct exit interviews following appointments.
