Mount Sinai Queens will develop a program to provide support, education, and guidance to patients and their caregivers in the inpatient medical and geriatric units.  This program grows out of a successful program in the emergency department.  Volunteers will provide help with understanding discharge plans, post-discharge referrals, and available resources, and introduce Next Step in Care materials to caregivers.  Volunteers will also serve, through the program coordinator, as liaisons between families, physicians, nurses, and support personnel.  The project will particularly focus on families with limited English proficiency.
