To support the development and implementation of a Web-based data collection tool that measures the health status, risk factors, and service utilization of NORC residents. Collected data will be analyzed to help design and target responsive service interventions and measure their effectiveness. Pilot results will be used to refine survey questions, and information collected will establish baseline data.
