To better serve high-cost Medicaid patients and reduce Medicaid spending on their care, through the Fund's Medicaid High-Cost Care Initiative. Following an initial year of research and planning conducted at five sites, the Initiative is now focusing on designing and testing demonstration projects. The Fund will assist with the submission of these tested projects to the State and foundations for subsequent support. The new grants fall into two categories:
• Bellevue Hospital Center and Montefiore Medical Center, both original Medicaid High-Cost Care Initiative consortium members, will each complete a more detailed analysis of their patient data, develop partnerships with relevant health, social service, and supportive housing providers, and design and staff care management teams that combine clinical, behavioral, and substance abuse expertise.
• Bronx-Lebanon Hospital Center and Woodhull Medical and Mental Health Center, both new consortium members, will use research methods and tools developed by the Medicaid High-Cost Care Initiative to conduct baseline research into their high-cost patient populations and plan responsive interventions.
