Family Caregivers as Partners in Health Care

To continue to support the Fund's Campaign for Family Caregivers, whose goal is to redefine how health care providers perceive and work with family caregivers, especially during transitions from one patient care setting to another. This grant will support 12-15 grants of $15,000 to $20,000 each to selected hospitals, nursing homes, and home care agencies to meet costs associated with the development and testing of various surveys and tools in support of family caregivers.
