This is the second grant in a project to develop a new care delivery model to achieve person-centered care, through new work and living environments, for the residents of the Isabella Geriatric Center and Cobble Hill Health Center. Specifically, this grant will support pilot units within the facilities as they implement their care plans, identify and respond to other organizational changes that may be necessary to facilitate the unit-based efforts and facility-wide changes, and monitor and assess the project.
