As part of the Fund's Medicaid High-Cost Care Initiative, these participating providers will create new service programs and redesign existing services to better serve high-cost Medicaid patients. Grants will support their analysis of Medicaid data to better understand patterns of service utilization and identify opportunities for intervention. Providers will also conduct interviews and focus groups with clinical staff, patients, and families; and develop relationships with community organizations and Medicaid managed care plans, with the goal of understanding how to serve this group of patients more efficiently and effectively. (A similar grant was awarded to Maimonides Medical Center in October 2005.)
